There’s a well-known statistic in Japan that gets repeated a lot in dental circles: the country has more dental clinics than convenience stores. With close to 70,000 clinics nationwide, many neighbourhoods in Tokyo, Osaka or Nagoya have several practices within a few minutes’ walk of each other. When patients have that many options, the clinic that shows up first on Google Maps often gets the booking, even if a better one is just around the corner.
Standing out online in Japan is not simple, though. Since the Medical Advertising Guidelines were tightened in 2018, clinic websites are treated as advertising, which means restrictions on patient testimonials, before-and-after photos and claims about treatment results. Many clinics had to rewrite their sites completely. At the same time, most practices depend on self-pay treatments like implants, aligners and ceramic crowns to stay profitable, and those are exactly the treatments that attract the most competition in search. Patients also check review and booking platforms such as EPARK before choosing, and in the bigger cities there’s a growing number of foreign residents looking for English-speaking dentists.
An agency that knows general SEO but not the medical advertising rules can create more problems than it solves. What clinics need is a team that understands both how Japanese patients search and what the guidelines allow.
We looked at agencies across Japan that work with dental clinics and chose the ones that impressed us most. Their experience in dental, their results, how clearly they price their services, and feedback from clinic owners all played a part in our decisions. You’ll find the list below.
Dental SEO Market Statistics: Japan
- Japan had 65,213 dental clinics at the end of March 2026, more than the roughly 56,700 convenience stores in the country (MHLW, May 2026 / JFA, April 2026).
- Closures now outpace openings: the number of dental clinics fell by 77 in March 2026 alone (MHLW, 2026).
- Tokyo has 10,511 dental clinics on its own, followed by Osaka with 5,303 and Kanagawa with 4,864 (MHLW, March 2026).
- Most practices are still small and owner-run: 46,953 dental clinics belong to individual dentists, against 17,550 run by medical corporations (MHLW, 2026).
- Dentist density varies twofold across the country, from 116.1 per 100,000 people in Tokyo to 55.9 in Aomori (MHLW, 2022 census, published December 2024).
- Japan is one of the few markets where Google is not dominant by a wide margin: it holds 55.43% of search, while Bing has climbed to 36.38% and Yahoo! Japan sits at 6.37% (Statcounter, April 2026).
- A dental SEO contract of around ¥50,000 a month often covers little more than a site check, Search Console review and one article, so clinics should confirm whether page improvements and MEO are included (Shika Lab, June 2026).
- Many SEO firms also charge a setup fee of ¥50,000 to ¥300,000 before monthly work begins (NTT Town Page, 2026).
- A mid-range dental clinic website with original design and basic SEO typically costs ¥500,000 to ¥1.5 million to build (Grill, 2026).
- Our research shows the biggest cluster of dental-focused web and SEO firms sits in Tokyo and Osaka, with a smaller group in Fukuoka (DentalSEOAgencies.com research).

How We Score Each Agency
We checked each one’s dental client work, public reviews, published results and terms before adding it, then read every client testimonial we could find. Only agencies that pass these checks are listed, so every DSA Score starts at 80. The other 20 depend on how many of the 100 points below each agency earns: DSA Score = 80 + points earned ÷ 5.
- Profile (up to 80 points). Five factors scored out of 20 each, then scaled to 80:
- Client reviews: a verified rating on Google, Clutch, G2, UpCity, Trustpilot or a similar platform
- Experience: years in business, a named founder or leader, and a dental or healthcare focus
- Client portfolio: how many dental clients the agency names publicly
- Published results: case studies with real figures, such as traffic, leads or revenue
- Transparency: published prices, clear contract terms, and a confirmed website and location
- Client feedback (up to 20 points). What clients actually say, from the testimonials we read on Google, independent review sites and the agency’s own website:
- Independent reviews (6): how many reviews we found that the agency doesn’t control
- Client satisfaction (8): how consistently clients praise the agency, adjusted so a handful of reviews can’t outscore a long track record
- Independent sources (4): how much of the feedback comes from Google and review sites rather than the agency’s own pages
- Dental clients (2): at least one review from a dental practice
- Verification. Client feedback points are reduced when reviews or agency details can’t be verified.
We summarise each client review in our own words and link to the original, so you can read it in full.
Price ranges are a guide based on published rates and typical fees for agencies of a similar profile. Always ask for a written quote.

Rather than handing over a finished site and walking away, they keep updating each website after launch, sparing busy dentists the job of managing content alone. Dental SEO in Japan: Frequently Asked Questions
How much does dental SEO cost in Japan?
Many contracts start around ¥50,000 a month, but at that level you often get little more than a site check, a Search Console review and one article. Many firms also charge setup fees of ¥50,000 to ¥300,000. Check exactly what’s included, especially page improvements and MEO (map optimisation).
What do the Medical Advertising Guidelines mean for my website?
Since 2018, clinic websites are treated as advertising. That means restrictions on patient testimonials, before-and-after photos without proper explanations, and claims about results. Many clinics had to rewrite their sites. Your agency should check every page against the guidelines.
How important is Google Maps (MEO) for Japanese clinics?
Very important. With so many clinics in cities like Tokyo and Osaka, patients often choose from the first few map results. A complete Google Business Profile, regular updates and genuine reviews can make a big difference to bookings.
Should I focus on self-pay treatments in my SEO?
Many clinics do, since implants, aligners and ceramic crowns bring in more revenue than insured treatments. These are also the most competitive searches, so they need detailed, accurate pages that stay within the advertising guidelines.
Is it worth creating English content for foreign residents?
In Tokyo, Osaka and other large cities, yes. The number of foreign residents is growing, and many search in English for dentists who can explain treatment in their language. A short English section with clear information can bring in patients few competitors are targeting.
